This section contains sample forms to be used by different groups at different times during the planning, implementation and evaluation of a Family Science program or community-based science program. The forms could be used as they are or could be changed to reflect the type of information the group is interested in gathering to inform future events. The data collected through these forms would allow the Planning Group to reflect on the unique needs of their community and avoid the program simply becoming a arranged of "check-off" events.
